How to register

Thank you for your interest in registering at our practice.

Please contact reception for information on how to register or you can complete the online form below. Please email the surgery a proof of identity and address to reception.knight@nhs.net for us to verify your details after completing the form.

Please make an appointment with one of our Nurse Practitioners for a Registration Interview.

    *Not all doctors are authorised to dispense medicines
    Please tell your family you want to be an organ donor. If you do not want to be an organ donor, please visit www.organdonation.nhs.uk or call 0300 123 23 23 to register your decision.
    I would like to join the NHS Blood Donor Register as someone who may be contacted and would be prepared to donate blood.
